Here's a good overview of how to approach minimally invasive plating of a femur. Diaphyseal fractures tend to heal via secondary processes with callus - so avoiding high strain is critical. If you can achieve an adequate length, alignment, and rotation with a few judicious clamps, there's no need to expose the periosteum and risk devascularization.

$We are least bothered about the gap when conservatively managing a diaphyseal fracture as long as its closed and there are no major risk factors affecting fracture biology. Here the initial gap at fracture site is 3-4 cm and persisted despite reducing further. Alignment was fine and gap got filled well with callus. Just an example of ways by which healing can happen.
